TRT Side Effects: What You Need to Know
Understanding TRT side effects is important. No medical treatment is without potential side effects, and testosterone replacement therapy is no exception. But here’s what most clinics won’t tell you: the vast majority of side effects from TRT are manageable, predictable, and avoidable with proper monitoring.
At Phoenix Men’s Health Center, we monitor every patient with regular lab work and follow-up visits. When changes are caught early, your provider adjusts your plan before issues develop. That’s the difference between supervised TRT and going it alone.
Common Side Effects of TRT
These are the side effects that occur most frequently with testosterone therapy. Most are mild, temporary, and manageable with dosage adjustments or supportive care.
Acne & Skin Changes
Increased oil production can lead to breakouts, especially on the back and shoulders. Usually temporary and manageable with skincare adjustments or dosage changes.
Fluid Retention
Some men experience mild water retention, particularly in the first few weeks. This typically resolves as your body adjusts to treatment. Monitoring helps catch it early.
Elevated Red Blood Cells
TRT can increase red blood cell production (polycythemia). This is why we monitor your hematocrit levels regularly. If levels rise too high, we adjust your protocol.
Mood Fluctuations
Some men experience temporary mood changes as levels adjust. This is typically short-lived and resolves as your body reaches a stable testosterone level.
Testicular Changes
Your body may reduce its own testosterone production while on TRT, which can cause decreased testicular size. Your provider can discuss options to address this if it's a concern.
Sleep Apnea
In some men, TRT may worsen existing sleep apnea. If you have a history of sleep apnea, make sure your provider knows. Monitoring and adjustments can help manage this.
Why Monitoring Is Everything
The difference between safe TRT and risky TRT comes down to one thing: monitoring. At Phoenix Men’s Health Center, every patient receives regular lab work and follow-up visits specifically designed to catch side effects before they become problems.
What we monitor: Total testosterone, free testosterone, estradiol, hematocrit, PSA, liver function, lipid panel, and metabolic markers. We don’t just check your testosterone number and call it a day. We look at the full picture.
How often: More frequently in the first 3-6 months as we dial in your protocol, then typically every 3-6 months once you’re stable. If something looks off, we adjust immediately.
What happens if a side effect occurs: We modify your dosage, change your delivery method, or add supportive treatments. Our providers, including Dr. Jean-Luc Le Provost and Dr. Rueda, have years of experience managing TRT protocols and know how to optimize your plan while minimizing side effects.
Long-Term Safety of TRT
The question of long-term TRT safety is one we take seriously. Here’s what the current research tells us:
Multiple large-scale studies have shown that medically supervised testosterone therapy, when prescribed to men with clinically diagnosed low testosterone, does not increase the risk of heart attack or stroke, as confirmed by a New England Journal of Medicine study. In fact, some research suggests that restoring testosterone to normal levels may have cardiovascular benefits.
Regarding prostate health, the outdated concern that TRT causes prostate cancer has not been supported by modern research. Current guidelines from major medical organizations do not list TRT as a cause of prostate cancer. However, men with active prostate cancer should not receive testosterone therapy, which is why we screen for PSA as part of our monitoring protocol.
The key takeaway: TRT is safe when it’s prescribed appropriately, dosed correctly, and monitored consistently. That’s exactly how we do it at Phoenix Men’s Health Center.
Have Questions About Side Effects?
Your $49 screening includes a provider consultation where you can ask anything about TRT, side effects, and monitoring. No pressure. Just answers.
Frequently Asked Questions
The most frequently reported side effects include acne, mild fluid retention, and elevated red blood cell count. These are typically manageable with dosage adjustments and regular monitoring.
No. Most side effects are temporary and resolve with dosage adjustments or when treatment is modified. If you stop TRT, side effects related to the therapy will resolve as your body returns to its baseline state.
TRT can potentially accelerate male pattern baldness in men who are genetically predisposed to it. If hair loss is a concern, discuss it with your provider. There are strategies to mitigate this while still receiving the benefits of TRT.
Yes. TRT can reduce sperm production, which may impact fertility. If you’re planning to have children, it’s important to discuss this with your provider before starting treatment. Our fertility services can help address this concern.
Regular lab monitoring, careful dosage titration, and ongoing follow-up visits. We track hematocrit, estradiol, PSA, and other key markers to catch any changes early. When something is off, we adjust your protocol immediately rather than waiting for symptoms to develop.
