Testosterone Injections vs Pellets vs Cream
There’s no single “best” delivery method for testosterone replacement therapy. The right choice depends on your lifestyle, your preferences, and how your body responds. At Phoenix Men’s Health Center, we offer all three options and help you find the one that fits.
Here’s an honest comparison so you can walk into your $49 screening already informed.
Testosterone Injections
Intramuscular or subcutaneous injection. Most common, most affordable, most precise dosing. Weekly or bi-weekly. Options for in-office or at-home self-injection after training.
Testosterone Pellets
Small pellets inserted under the skin during a quick in-office procedure. Steady hormone release for 3-6 months. Set it and forget it. Higher per-visit cost, fewer visits.
Creams & Gels
Daily topical application absorbed through the skin. Non-invasive, flexible dosing, easy to adjust. Requires daily consistency. Must avoid skin contact transfer to others.
Testosterone Injections: The Details
Testosterone injections are the most widely used TRT delivery method, and for good reason. They offer precise dosing control, fast absorption, and are the most cost-effective option.
How it works: Testosterone cypionate or enanthate is injected into the muscle (typically the thigh or glute) or subcutaneously. Your provider determines the dose based on your lab work.
Frequency: Typically once per week or every two weeks. Many of our patients learn to self-inject at home after training in our office, which means fewer clinic visits.
Pros: Most affordable option. Precise dose control. Fast-acting. Well-researched with decades of data. Can be done at home.
Cons: Requires needles (some men prefer to avoid this). Testosterone levels can peak and trough between injections, though proper scheduling minimizes this. Injection site soreness is possible but usually mild.
Best for: Men who want the most control over their dosing, are comfortable with self-injection, and want the most cost-effective option.
Testosterone Pellets: The Details
Testosterone pellets are small, rice-grain-sized pellets that are inserted under the skin during a quick in-office procedure. They release a steady stream of testosterone over 3-6 months.
How it works: Your provider makes a tiny incision (usually in the hip area), inserts the pellets, and closes it with a small bandage. The procedure takes about 10-15 minutes. You walk out and don’t think about your testosterone for months.
Frequency: Every 3-6 months depending on how quickly your body metabolizes the pellets.
Pros: Most consistent testosterone levels (no peaks and troughs). Minimal maintenance. No daily routine to remember. No needles between visits.
Cons: Higher per-visit cost. Requires an in-office procedure for each insertion. Not easily adjustable once inserted. Small risk of pellet extrusion (rare).
Best for: Men who want a set-it-and-forget-it approach, don’t want to deal with daily or weekly dosing, and value consistency over flexibility.
Testosterone Creams and Gels: The Details
Topical testosterone is applied daily to the skin, where it’s absorbed into the bloodstream. It’s a non-invasive option that many men find convenient.
How it works: You apply a measured dose of testosterone cream or gel to your shoulders, upper arms, or inner thighs each morning. The testosterone absorbs through the skin over several hours.
Frequency: Daily application, typically in the morning.
Pros: No needles. Easy to apply. Dosing is flexible and easy to adjust. Maintains relatively steady testosterone levels with daily use.
Cons: Requires daily consistency (miss a day, levels drop). Risk of skin-to-skin transfer to partners or children if application area isn’t covered or washed. Some men experience skin irritation. Absorption rates vary between individuals.
Best for: Men who prefer a non-invasive daily routine, want flexible dosing, and don’t have concerns about skin transfer to household members.
How to Choose the Right Method
There’s no wrong answer here. The best method is the one you’ll actually stick with consistently. Here are the questions to ask yourself:
How much do I want to think about it? If you want minimal maintenance, pellets are your answer. If you don’t mind a daily routine, creams work great. If weekly is your sweet spot, injections are the way to go.
Am I comfortable with needles? If not, creams or pellets eliminate that concern entirely.
What’s my budget? Injections are typically the most affordable. Pellets have a higher per-visit cost but may save time. Creams have a consistent monthly prescription cost. Read more about TRT costs in Phoenix.
Do I live with a partner or young children? If skin transfer is a concern, injections or pellets may be a safer choice than topical creams.
Your provider will help you weigh all of these factors during your first appointment. The goal is finding the method that fits your life so you stay consistent with treatment.
Not Sure Which Method Is Right for You?
That’s exactly what the consultation is for. Book your $49 screening and your provider will recommend the delivery method during your follow-up consultation that makes the most sense for your body and your schedule.
Frequently Asked Questions
Injections typically reach peak testosterone levels the fastest. Pellets provide a slower, steadier release. Creams maintain relatively consistent levels with daily use. Your provider will discuss timing expectations for each method.
Yes. Many men start with one method and switch as they learn what works best for their lifestyle. Your provider can help you transition between delivery methods smoothly.
Side effect profiles are similar across all methods since they all deliver the same hormone. The main differences are method-specific: injection site soreness for injections, skin irritation for creams, or minor insertion site discomfort for pellets. Read more about TRT side effects.
Yes. Many of our patients self-inject at home after being trained by our staff. We teach proper technique and provide everything you need. Some men prefer to come into the office for their injections, and that’s fine too.
Pellets typically have a higher per-visit cost but require fewer visits (every 3-6 months vs weekly). Injections have a lower per-visit cost but more frequent visits. Over a year, the total cost may be comparable depending on your protocol. See our full cost breakdown.
